前沿高流量鼻导管氧疗治疗拔管后急性低氧性

致：在一项大样本多中心非劣效随机实验中，Stéphan医生及其同事发现[]，高流量鼻导管氧疗预防或医治心胸外科手术拔管后急性低氧性呼吸功能衰竭与无创通气一样有效。高流量鼻导管氧疗是新出现的氧疗技术，具有很多生理及临床优点[]。但是，根据上述研究得到确切结论前，应当注意一些重要的问题。

基于研究的入选标准，根据3种不同的策略实行无创通气或高流量氧疗：()自主呼吸实验（作为脱机或拔管技术）失败后（初期拔管）；()拔管后急性呼吸衰竭的高危患者自主呼吸实验成功后（预防性）；()自主呼吸实验成功但拔管失败（医治性）[]。这1研究的结果并未根据上述不同临床适应症进行分层或报告。

拔管后使用无创通气或其他呼吸支持医治措施时，临床医生需要初期辨认上述3种情况，由于各自的医治目的不同。据我们所知，还没有随机临床试验评价外科患者自主呼吸实验失败后初期拔管及无创通气的策略。

在术后患者，拔管后预防性无创通气研究结果其实不一致[4]。相反，很多研究显示，拔管后医治性无创通气对患者预后（包括病死率）的有益作用较标准氧疗更加显著，特别对胸外科手术患者[5]。

多数研究采取了不同的无创通气方式（CPAP，PSV或BiPAP）[5]。这就提出以下的问题，即采取间歇无创通气（PSV或BiPAP）而不是传统上为大家接受的常规医治（如标准氧疗）作为对比组，评价高流量鼻导管氧疗的效果。
